BTSA Consent Form - In order to participate in the BTSA Program to receive a Clear Professional Credential, participants must complete and sign an agreement that outlines their responsibilities. This must be done within 120 calendar days of the start of the initial teaching contract.
Early Completion Option - Certain exceptional, experienced candidates may be eligible for this modified BTSA program option. If you have taught in a private school for 2 or more years; or if you have taught out of state; or if you have had two years of participation in an intern program, you may be eligible for this option. Please read the requirements carefully.
Extension Request - Special consideration will be given to extenuating circumstances due to a family emergency or health conditions beyond the control of the PT that impede the possibility of completing program requirements within the two-year period. A request for extension may be submitted.
Clear Credential Application - Upon successful completion of the BTSA Induction program, the participant will complete the Clear Credential Applicatiuon and the Consortium Coordinator will submit the application using the secure online process.
Collaborative Log: This form is available for those who prefer to use the computer to complete the logs rather than the NCR forms.
